Once you've called for tuition rates, toured our classrooms and decided to make your child part of the CFC family, please provide us with the following:

   1.  Completed Enrollment Form (download below)

   2.  Child's Immunization Record (get copy from doctor)

These forms may be delivered in person or emailed to admin@cfccu.hrcoxmail.com
